When you confirm the appointment time with SOS Chinese Medicine Clinic, it represents that you are aware of and accept the following terms.
當你與蘇安莎中醫診所確認預約時間時,即代表你知悉並確認以下條款。

就診注意事項

  1. 醫療機構屬於高風險場所,診所範圍內必須佩戴口罩。

  2. 如病人於約定到診時間起計5分鐘內未到達診所會被視爲遲到,將被安排在現場首3位病人之後就診。醫師會在不影響原本預約病人的原則下盡快安排就診,敬請耐心等候。

  3. 診所只接受現金/EPS/轉數快付款。

醫療相關事項

  1. 就醫時請告知醫師會引起您藥物過敏的藥物或食物的名稱、種類。

  2. 如需針灸治療,到診前請先進食少量輕食,避免針灸不適/暈針。

  3.  (女性病人適用)如因診症需要病人檢驗懷孕狀態,診所備有驗孕棒供病人購買。病人可以在診所即時購買及進行檢驗 (以診所最新價格爲準)。

處方、收據及病假證明書安排

  1. 診所處方 (如有) 及收據一經發出後不能再修改任何信息,請病人取藥時核對清楚信息無誤。

  2. 醫師會按處方藥物的患者者簽發病假證明書,如有需要請在診症時跟醫師溝通,診症過後系統不能補發。

惡劣天氣安排 (八號風球或黑色暴雨警告)

  1. 開診時間 前 懸掛:
    診所會暫時停止應診,求診者毋須前往本診所應診。

  2. 在開診時間 內 懸掛:
    本診所將按情況逐步暫停應診並通知求診者,求診者應考慮當時天氣及交通情況,始決定是否前往診所。

  3. 在開診時間結束前已 除下:
    在正常情況下,診所將於信號除下的兩小時後恢復應診,信號除下兩小時後的預約將會照常應診。
    建議求診者先考慮當時天氣情況及可供使用的交通服務等各因素,在安全的情況下前往本診所應診。

  4. 因惡劣天氣而無法求診者,診所會主動聯絡安排重新預約。

Consultation Notice

  1. Medical institutions are high-risk places and masks must be worn within the clinic.

  2. Patient not arriving on time ( within 5 minutes of the confirmed booking timeslot) will be considered late and the consultation will be arrnaged after the first 3 patients on clinic who are arriving ontime.  We will arrange your consultation as soon as possible without affecting the scheduled appointment of the other patients.  Your paitent is highly appreciated.

  3. Our clinic only accepts cash/EPS/FPS payment.

Treatment Precautions

  1. During the consultation, please inform your doctor of the name and type of the medicine or food that may cause your allergic reaction.

  2. If you are going to have acupuncture treatment, please have a light and small size of meal before having the treatment so as to avoid acupuncture discomfort/fainting.

  3. (Applicable to female patients) Female patients would be required to  confirm the pregnancy status during the consultation, the clinic offers pregnancy test sticks for purchase. You can purchase and tests immediately at the clinic (subject to the clinic’s latest price).

Arrangement of prescription, receipt and sick leave certificate

  1. Once the clinic prescription (if any) and receipt are issued, no information can be modified. Please check the information is correct and in order when picking up the medicine.

  2. Our Chinese Medicine Practitioner will issue a sick leave certificate to the patient depending the patient’s illness and health condition. If sick leave certificate is required, please communicate with our Chinese Medicine Practitioner during the consultation. Our clinic system cannot issue it after the consultation.

Adverse weather arrangements (Typhoon signal No. 8 or black rainstorm warning)

  1. Issued before opening time:
    Outpatient services will temporarily suspend. Patients do not need to go to the clinic for consultations.
  2. Issued during clinic hours:
    will gradually suspend according to the situation. Patients will be notified and  should consider the prevailing weather and traffic conditions before deciding whether to go to the clinic.

  3. Removed before the end of clinic hours:
    Under normal circumstances, the clinic will resume consultations two hours after the signal is removed. Appointments made two hours after the signal is removed will be available as usual. It is recommended that patients first consider various factors such as weather conditions and available transportation services at the time, and then go to our clinic for consultation when it is safe to do so.
  4. For those who are unable to seek treatment due to bad weather, the clinic will proactively contact you to arrange rescheduling.
